HEALTH SERVICES GUIDELINES AND PROCEDURE

  1. Patients with maintenance drugs or special needs must present medical certificate from their attending physician for record purposes.
  2. Students with contagious illness should be isolated or be confined at home (e.g. chicken pox, measles, mumps) to prevent spreading the disease.
  3. Students treated with communicable diseases should seek clearance from the clinic staff if they can go back to class.

BEFORE THE TREATMENT:

  1. The teacher who sent the student to the clinic will notify the parents/guardian immediately.
  2. The patient should be accompanied by his/her teacher or classmate on the way to the school clinic.

DURING THE TREATMENT:

  1. Upon receiving the patient, he/she will be assessed by the clinic personnel on duty and will be given the necessary first aid.
  2. Patients are only allowed to stay and rest in the clinic if they are sick and may stay in the clinic until their condition is stable and when vital signs are normal.
  3. A necessary “starting dose” is given to patients. Antibiotics are prescribed only by the school physician.

AFTER THE TREATMENT:

  1. The nurse/doctor on duty will provide a medical certificate when the patient is discharged so that the teacher/parent will be informed on what medication was given, what time is the next dose and what are the interventions given to patient.

IN CASE OF EMERGENCY AND REFERRAL:

  1. The patient will be assessed by the clinic personnel on duty and will be given the necessary first aid. The nurse on duty will immediately inform the principal/dean/adviser.
  2. The principal/dean/adviser will immediately notify the parents/guardian.
  3. The patient will be sent immediately to the nearest hospital together with the teacher/dean/principal so that medical treatment will be given.
  4. The teacher/dean/principal who accompanies the patient to the hospital will not leave him/her unless tha patient’s parent/guardian or significant others arrived and the patient has been properly endorsed to the hospital staff.
  5. An incident report will be made by the adviser/teacher for proper documentation of the incident.

ON ISSUANCE OF EXCUSE SLIP:

  1. The clinic will issue medical certificates to employees and students only when they have undergone proper medical consultation with the clinic staff.
  2. Students can be excused from class and will be allowed to go home only with consent from parents/guardian.
